Psychologist vs Psychiatrist vs Psychotherapist in India

By the Dandelion Reflect team

Last updated:

Psychologist, psychiatrist, psychotherapist or counsellor? Who can prescribe, which titles are regulated in India, and who to see for anxiety, low mood or a crisis.

TL;DR: A psychiatrist is a medical doctor and the only one of the four who can prescribe medicine. A clinical psychologist is defined in Indian law and registered with the Rehabilitation Council of India (RCI); they assess, diagnose and offer therapy, but don't prescribe. "Psychotherapist", "counselling psychologist" and "counsellor" aren't protected titles in India, so check the person's training and supervision. If you're thinking about harming yourself, skip the comparison and call Tele-MANAS on 14416, free and 24x7.

You type "psychologist near me" and get psychiatrists, counsellors, psychotherapists and life coaches in the same list. The titles get used loosely in daily speech. The differences matter most on two questions: can this person prescribe medicine, and can you check their registration?

This guide answers those two questions and then gets practical about who to see for what. For the wider process of finding a good fit, including questions for an intro call and red flags, see how to choose a therapist in India.

The Short Answer in One Table

The two legal definitions come from the Mental Healthcare Act, 2017, section 2(1)(g) for a clinical psychologist and section 2(1)(y) for a psychiatrist.

Psychiatrist: A Doctor Who Can Prescribe

A psychiatrist has a medical degree and then a postgraduate degree or diploma in psychiatry. They can diagnose mental illness, order tests, prescribe and adjust medicine, and admit someone to hospital when that's needed. Their registration shows up on the National Medical Commission's Indian Medical Register, which lists doctors registered with state medical councils.

Some psychiatrists offer therapy as well. In practice, many appointments, especially follow-ups, are short and focused on how you're doing on the medicine. If you want weekly talking work alongside it, you'll often see a therapist too.

Clinical Psychologist: Assessment, Diagnosis and Therapy

Under the Mental Healthcare Act, a clinical psychologist has a clinical psychology qualification recognised by the RCI, or a postgraduate degree in psychology plus a two-year, full-time MPhil in Clinical Psychology with supervised clinical training. They enrol on the RCI's Central Rehabilitation Register and should be able to give you their CRR number.

Clinical psychologists are trained in psychological testing. If a school, employer, court or doctor has asked for a formal assessment (for ADHD or a learning disability, for example), this is usually who you need. Many also offer therapy. They can't prescribe.

Psychotherapist and Counselling Psychologist

These two titles have no legal definition in India. A psychotherapist might have a master's degree in psychology followed by years of supervised training in an approach such as Transactional Analysis, CBT or Emotionally Focused Therapy. Someone else with the same title might have done a short online course. The title alone doesn't tell you which.

So ask. What's your degree? What approach did you train in, for how long, and with which body? Who supervises your work? A trained psychotherapist answers those questions easily. If you want to understand what the work itself involves, how psychotherapy works walks through it.

Psychotherapists don't prescribe medicine. A psychotherapist isn't a doctor, unless they also hold a medical degree, which is rare.

Counsellor

"Counsellor" is the broadest title. Some counsellors hold a master's in counselling psychology and work much like psychotherapists. Others have brief training and offer support and guidance rather than longer-term therapy. Schools, colleges, NGOs and helplines often use the word for staff who listen and help you work out next steps, and that can be exactly what you need at the start.

Who to See for What

These are starting points, not rules. A good professional will tell you if someone else would suit you better.

What's going onA sensible first step
Thoughts of ending your life or harming yourselfUrgent help first: Tele-MANAS (14416), your nearest hospital emergency department, or a psychiatrist the same day
Worry, overthinking or panic that's getting in the way of daily lifeA therapist; add a psychiatrist if it's severe or isn't easing
Low mood with poor sleep, appetite changes, or trouble getting out of bed for weeksA psychiatrist or GP to check whether medicine would help, plus a therapist
Hearing or seeing things others don't, or very fast, racing thoughtsA psychiatrist, soon
A formal assessment or report someone has asked forA clinical psychologist
Relationship problems, the same fight on repeatA therapist with couples training
Grief, burnout, a big life change, patterns you keep repeatingA therapist

For anxiety specifically, what to expect from therapy for anxiety covers how that work usually goes. For low mood, depression therapy online does the same.

Can You See a Psychiatrist and a Therapist Together?

Yes, and many people do. The WHO's depression fact sheet lists psychological treatment as the first option for depression and adds antidepressants for moderate to severe depression. When both are involved, the psychiatrist manages the medicine and the therapist sees you weekly. With your permission, they can talk to each other.

Starting medicine doesn't mean therapy has failed, and starting therapy doesn't commit you to medicine. Some people start with one and add the other later.

How to Check a Registration

  • Psychiatrist: search their name or registration number on the NMC Indian Medical Register.
  • Clinical psychologist: ask for their CRR number and search it, or their name, on the RCI register search.
  • Psychotherapist or counsellor: there's no register to search. Ask about their degree, their training in a named approach, and who supervises them.

If someone calls themselves a clinical psychologist and can't give you a CRR number, ask why.

Why the Gap Matters

The National Mental Health Survey 2015-16 found a treatment gap above 60% for every mental disorder it studied except epilepsy. Confusion about who to see is one small part of that. Cost is another, and the guide to the cost of therapy in India lays out sourced prices for each kind of professional.

Before a first appointment, it can help to write down what's been happening: when it started, what makes it worse, how you're sleeping. A few lines a day in a journal such as Dandelion Reflect gives you something concrete to bring. Whoever you see will ask.

Dandelion Psychotherapy, the practice that publishes this blog, offers online talk therapy for individuals and couples. It doesn't prescribe or do formal assessments. If talk therapy sounds right, the first step is a free 15-minute intro call, where you can ask about training and supervision directly.

Frequently Asked Questions

Is a psychotherapist a doctor?

No, unless they also have a medical degree. A psychotherapist offers talk therapy and can't prescribe medicine. A psychiatrist is a medical doctor and can prescribe.

Should I see a psychiatrist or a psychologist for anxiety?

For most anxiety, start with a therapist: a clinical psychologist, a psychotherapist or a counselling psychologist. See a psychiatrist as well if the anxiety is severe, if panic attacks are stopping you from working or going out, or if therapy alone isn't helping after a fair try.

What is the difference between a psychologist and a counsellor?

In India, "clinical psychologist" is a legally defined title with RCI registration and training in assessment and diagnosis. "Counsellor" isn't regulated and covers a wide range of training. A well-trained counsellor can be very helpful; check their training rather than the title.

Can a psychologist prescribe medicine in India?

No. Only medical doctors can prescribe, so for psychiatric medicine you need a psychiatrist, or a GP for some common medicines.

Do I need a referral to see a psychiatrist or therapist?

Usually not. In private practice you can book a psychiatrist or a therapist directly. Some government hospitals have their own registration process at the OPD.
